Because we are sole practitioners, fees are rendered to each individual practitioner. 

Psychological Services (Dr. Rebecca Fineman, Dr. Lori Bond ):

Evaluation sessions (parent intake, school/home visit, testing, parent conference regarding assessment findings) are billed at $350 per hour.  Scoring of psychological tests and rating forms are billed at $350 per hour.  This fee also covers brief telephone conversations with your child’s pediatrician or teacher, review of records, travel time within half an hour of the office and report writing.  Clients will be billed for time spent conducting phone calls that are clinical in nature and last longer than 10 minutes.  A rate of $350 per hour (evaluation) and $250 per hour for therapy is charged for phone calls over 10 minutes.

Therapy sessions are typically 50 minutes in length and are billed at $250 per session.

Speech and Language Services (Amy Travers MS, CCC-SLP):

Evaluation sessions are billed at $250 per hour.  Hourly rates are charged for the following:  School visits or home visits; testing; scoring of tests and transcription of language samples; disposition among Mosaic clinicians in order to integrate findings and formulate recommendations; and meeting with parents/guardians to share evaluation results.  The evaluation fee INCLUDES report writing, travel within 30 minutes of the office, collateral phone calls up to 10 minutes each, and record review.  Clients will be billed for time spent conducting phone calls that are clinical in nature and last longer than 10 minutes.  A rate of $250 per hour (evaluation) and $170 per hour for therapy is charged for phone calls over 10 minutes. 

Therapy sessions are typically 50 minutes in length and billed at $170  per session.

Travel over 60 minutes round trip from the office is charged at $250 per hour for the duration of the trip.

Occupational Therapy Services (Elizabeth Zias M.A.Ed, OTR/L, SWC):

Evaluations are billed at $250 per hour.  Hourly rates are charged for the following:  School visits or home visits; testing; scoring of tests; disposition among Mosaic clinicians in order to integrate findings and formulate recommendations; and meeting with parents/guardians to share evaluation results.  The evaluation fee INCLUDES report writing, travel within 30 minutes of the office, collateral phone calls up to 10 minutes, and record review.

A rate of $250 per hour (evaluation) and $170 per hour for therapy is charged for phone calls over 10 minutes .  Travel over 60 minutes round trip from the office is charged at $250 per hour for the duration of the trip.

For all Mosaic clinicians, e-mail will be used solely for the purpose of scheduling or at the clinician's and client's mutual discretion.  E-mail or phone messages regarding scheduling will not be billed.
 

Insurance Reimbursement

If you plan to request reimbursement from your insurance, please check your coverage carefully and contact your plan administrator if you have questions.  Mosaic clinicians do not contract directly with any insurance companies and do not accept payment from insurance companies.  Some carriers will only pay for therapists pre-approved by them, will pay at a lower rate for non-approved providers, will only pay for mental health services that are pre-approved or will not reimburse for some conditions or diagnoses.  If you desire, you will be provided with a statement each month which can be submitted to your insurance company.
